how to get Intervention name and the Record date


Welcome to part 3 of our 3 part series in the exciting world of nursing data.  In this segment I’ll focus on getting the Intervention name and the Record date.  Part 3 approaches the Nursing Documentation from the angle of an Intervention which is a different angle than parts 1 and 2 approached the data.  With that in mind, we’ll gather the data starting from the base table of Interventions and “join” our way down to the queries table.

A few vocabulary terms and relationships to know: 

                An Intervention is an activity performed with the patient.  The activity we are concerned with is, the Assessment.  An Intervention can have one or more Assessments.  It is during an assessment that patient’s information is gathered and documented.   An Assessment is a collection of questions that must be answered for this Intervention.   Assessments can consist of one or more Documentation Sections.  A Documentation Section is a subset of questions under the Assessment that generally will pertain to a particular subject, like cognitive function for example.  The Documentation Sections allow for grouping the questions of an assessment into logical bunches.  A Document Section can be broken down into one or more Question Sets. A Question Set consists of one or more queries or questions.  These questions are ordered within the Question Set.

Timing is often a factor when looking at Interventions.  You may want the information from the first occurrence of an Intervention or perhaps from the last occurrence of that same Intervention.  The InterventionActivityUrnID is the field that helps you accomplish this.  Additionally, the InterventionActivityChangeUrnID is used to find edits to responses to the questions.  The highest InterventionActivityChangeUrnID is the most recent edit.  By ignoring the InterventionActivityChangeUrnID you can get an audit trail and see the progression of documentation.

Code

                The approach taken here is to find the intervention documentation for a single patient.  However with minor adjustments to the code you can use this to find all documentation on a particular intervention for a period of time.  Note how the JOINs in the code mirror the Structure Summary and the flow and relationships of the vocabulary terms.

Now you have the basics for getting Nursing Data and sample code from which to start.  As you can see it takes a number of tables to get to the Intervention data. Some reports want the ActivityDateTime rather than the RecordDateTime.  You can use an ORDER BY on the main query to see the results in the order that makes sense for your report.  If you run into any problems or have questions, feel free to email me at kmacpherson@acmeware.com.
